The Case Manager coordinates ancillary services such as housing, medical care, and employment to help clients achieve their individual treatment plans. They assess client needs, maintain records, and collaborate with primary counselors to monitor progress and ensure continuity of care.
Applicants must have a high school diploma, a valid Ohio Driver License with two years of experience, and proof of automobile insurance. A CDCA credential is preferred, and candidates must pass a BCI check and drug screen.
Alcohol and Drug Freedom Center of Knox County is seeking a team focused, Case Manager to work full-time with clients. The Case Manager provides coordination and referrals for clients who need ancillary services to help them achieve their individual treatment plans. This includes assisting clients with housing, medical care, food, employment, and transportation in addition to working with other community organizations to provide continuity of care through collaborative services and information sharing.

This is an office and community-based position that involves some travel and transporting of clients within the county. Because of this, a valid Ohio Driver License and two years of driving experience are required. Case Management Specialists must maintain and show proof of full automobile insurance coverage.
Qualified applicants are required to have a high school diploma or equivalent. CDCA (Chemical Dependency Counselor Assistant) credential preferred (expected to be obtained within one year of employment if not already obtained). Applicants must be able to pass a BCI check and post offer/pre-employment drug screen.

If alcohol, drug use, or mental health issues are affecting your work, family or health or you are worried about the drug or alcohol use of a loved one The Freedom Center offers hope, understanding and a new beginning. We can help you decide if alcohol or other drugs are hurting you and those you care about. We will also help you explore ways you and your family can get better. You and your counselor will develop a treatment plan together to meet your needs in a way that works for you. We encourage taking good care of yourself and overall wellness. We will help you identify your strengths to use as tools in the recovery process. It will be important for you to develop good supports with your family and community to be sober and independent.
