The Case Manager provides community-based support to individuals with severe and persistent mental illness by assessing needs and developing individualized service plans. They assist clients across various life domains, including housing, health, and vocational needs, while coordinating community resources.
Requires a Bachelor's degree in Social or Health services and at least three years of experience working with adults with psychiatric disabilities. Candidates must have Suicide Prevention and Non-Violent Crisis Intervention training, a valid driver's license, and a Vulnerable Sector Check.
