The Nurse Care Manager provides health screenings, assessments, and medical support for medically vulnerable clients across the agency. They are responsible for health coordination and wellness services within a collaborative, mission-driven recovery setting.
Candidates must hold a current and unrestricted Nursing License in the State of Michigan (LPN considered). Preferred experience includes behavioral health, substance abuse treatment, and expertise in wound care and chronic disease management.
With MMRS, you will play a vital role providing health screenings and assessments, health coordination, medical support and wellness services, for medically vulnerable clients, across the agency. This role is ideal for a motivated healthcare provider who thrives in a collaborative, mission-driven setting and seeking growth.
