Are you passionate about helping people live their healthiest lives? Do you thrive in a dynamic, supportive environment where your contributions truly matter? If so, Medcan is the place for you!

This job is for a current vacancy.

The Role

The Nurse Consultant serves as a trusted clinical resource and primary point of contact for clients within Medcan's Year Round Care program. This role is ideal for an experienced nurse with strong clinical assessment and critical thinking skills, exceptional communication abilities, and a passion for delivering outstanding client care.

Working primarily through telephone, email, and client portal communication, the Nurse Consultant provides education and support, facilitates ongoing case management, and supports specialist referrals and diagnostic studies in collaboration with the Referrals and Medical Records teams. The role collaborates closely with physicians, nurses, coordinators, and allied health professionals to ensure a seamless client experience.

What You'll Do

  • Communicate and explain routine, abnormal, sensitive, and critical clinical results based on physician review and recommendations.

  • Serve as the primary nursing contact and trusted resource for clients throughout their healthcare journey.

  • Conduct telephone-based nursing assessments and symptom triage using sound clinical judgment.

  • Determine the urgency and appropriateness of client concerns and provide guidance regarding physician follow-up, urgent care, or emergency services when required.

  • Provide health education and support related to prevention, healthy lifestyle practices, treatment plans, and new or ongoing diagnoses.

  • Support clients and their families through education, counselling, and navigation of healthcare and community resources.

  • Support specialist referrals and investigative studies in collaboration with the Referrals and Medical Records teams.

  • Collaborate with physicians, nurses, coordinators, and allied health professionals to support optimal client outcomes.

  • Coordinate appointments, prescription renewal requests, and ongoing case management activities.

  • Navigate and de-escalate complex client concerns, escalating issues through the appropriate clinical, operational, or client experience pathways when required.

  • Maintain accurate, thorough, and timely documentation within electronic medical records and client management systems.

  • Partner with colleagues across Membership teams to support resource sharing and exceptional service delivery.

  • Participate in rotational evening, weekend, and after-hours on-call coverage.

What You'll Need

  • Registered Nurse in good standing with the College of Nurses of Ontario.

  • Current Basic Life Support (BLS) or CPR Healthcare Provider certification.

  • Minimum 2-5 years of nursing experience, preferably in case management, primary care, ambulatory care, community nursing, or chronic disease management.

  • Demonstrated clinical assessment, triage, critical thinking, and decision-making skills.

  • Exceptional written and verbal communication skills with the ability to communicate complex clinical information clearly and compassionately.

  • Strong documentation skills with meticulous attention to detail.

  • Excellent organizational and time management skills with the ability to prioritize effectively in a fast-paced environment.

  • High emotional intelligence, strong problem-solving abilities, and a client-focused, hospitality-driven approach to care, including the ability to navigate and de-escalate complex client concerns.

  • Experience using Electronic Medical Records (EMRs), preferably Accuro, and Client Relationship Management (CRM) systems such as Salesforce.

  • Strong technical proficiency with Microsoft Teams, Outlook, Word, Excel, PDF editing software, web-based applications, internet browsers, and client portal technology.

  • Ability to work independently while collaborating effectively within a multidisciplinary team.

This is an 19-month full-time (40 hours/week) contract position with hours scheduled primarily during business operating hours, Monday through Sunday.  We are seeking a Nurse Consultant to fill a Tuesday to Saturday shift from 12:30 p.m. to 9:00 p.m. The successful candidate will participate in a shared after-hours on-call rotation approximately every 6-8 weeks.

While the role is primarily remote, the team aims to meet once per month at our 150 York Street location for team discussions, collaboration, professional development, and team-building activities.
