Forensic Case Manager
Position Summary:The Forensic Case Manager is responsible for providing comprehensive case management services to justice-involved individuals referred to Lightshare Behavioral Wellness and Recovery. This role bridges the behavioral health and criminal justice systems, ensuring clients receive coordinated care, compliance monitoring, and support as they navigate court mandates, treatment plans, and community reintegration. The Case Manager promotes rehabilitation, accountability, and stability while advocating for the client’s wellness and public safety.
Qualifications:
Bachelor’s degree in Social Work, Criminal Justice, Psychology, Counseling, or related field required; Master’s degree preferred.
Minimum of 2 years’ experience in forensic case management, probation services, or behavioral health.
Knowledge of criminal justice processes, behavioral health systems, and evidence-based practices in recovery.
Strong interpersonal and communication skills; ability to engage individuals from diverse backgrounds.
Ability to manage a caseload with competing priorities in a fast-paced environment.
Proficiency in documentation, case notes, and electronic health records.
Responsibilities:
Conduct intake assessments, including psychosocial history, risk/needs evaluations, and service planning for justice-involved clients.
Develop individualized case management and recovery plans aligned with legal requirements, treatment goals, and client strengths.
Monitor compliance with court orders, probation/parole conditions, and treatment engagement, providing timely reports to supervising authorities.
Facilitate access to behavioral health, substance use treatment, housing, employment, and educational resources.
Maintain consistent communication with probation officers, attorneys, judges, treatment providers, and other stakeholders.
Provide crisis intervention, advocacy, and motivational support to encourage client accountability and progress.
Document case activity in accordance with agency policies, HIPAA, and legal standards.
Participate in multidisciplinary team meetings, case conferences, and court hearings as required.
Track outcomes and provide data for program evaluation and quality improvement.
