This posting serves as a general application for current and future therapy opportunities throughout our service area. We are seeking licensed Physical Therapists (PTs), Occupational Therapists (OTs), and Speech-Language Pathologists (SLPs) who are passionate about delivering high-quality, patient-centered care in the home. General Summary
Provides skilled therapy services to patients in their place of residence in accordance with the physician's plan of care and professional standards of practice. Evaluates, develops, and implements individualized treatment plans designed to maximize each patient's functional abilities, independence, safety, and overall quality of life. Collaborates with physicians, caregivers, and the interdisciplinary care team to monitor progress, educate patients and families, and adjust treatment plans as needed while maintaining accurate and timely clinical documentation.
This summary is broad enough to apply to PT, OT, and SLP positions while still accurately reflecting the responsibilities of home health therapists.
Patient Population: Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations.
Essential Functions:
Assist clients with tasks within the scope of job description based on the Plan of Care (POC).
Communicate effectively with administrative staff regarding client/family needs assessments, including reporting any significant changes in client condition.
Provide care as outlined in the Care Plan and accurately document the cares provided.
Contribute to continuity of care, effective communication between all care team members, and regular participation in Case conferences/Team Meetings.
Apply concepts of infection control and universal precaution in coordination/performing client care activities to protect both staff and patients.
Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency. Completes training or in-services in a timely manner as assigned by your supervisor.
Meet mandatory continuing education requirements of the Agency/licensing board.

Requirements
Experience:
(1) year experience as a therapist in a home health setting. Successful completion of formal training in nursing arts may be substituted for the qualifying experience, subject to applicable State law, regulation, or rules.
Education:
High School Graduate or equivalent
Current CPR certification.
Current driver’s license in good standing. The employee is responsible for renewing their driver’s license before it expires to continue employment.
Reliable transportation and auto liability insurance.
On-going employment as an HHA always requires the maintenance of a valid HHA certificate in the state where the caregiver practices and a valid CPR certificate. The employee is responsible for renewing these before they expire to continue employment.
Physical Requirements:
Prolonged standing/walking is required.
The ability to lift/transfer up to 50 lbs.
Requires working under stressful conditions to meet deadlines and patient needs and make quick decisions and resource acquisition.
