* This is a limited-term role with an anticipated timeline of 6–8 months and the possibility of extension based on business needs.  Benefit eligibility is determined in accordance with plan provisions and employment status.  

All CM Best Practice Leads (BPLs) contribute to excellence in care management practice by supporting the vision, quality standards, and practice development of Interdisciplinary Team (IDT) staff. The Best Practice Leads guide and support care management practice through being a resource to LCI staff in areas such as care planning, coordination of care, and conducting onboarding and training. This role is responsible to assist in the development and deployment of policy, procedure, processes, and guidelines specific to care management practice in accordance with but not limited to the Department of Health Services (DHS) Family Care Contract, DHS directives, and technology changes/updates.

This CM BPL Education Specialist role provides a deeper knowledge of and supports comprehensive, engaging, and effective adult learning techniques and deployment of content within the Care Management Division. This position is the primary CM BPL team member responsible for creating and managing access to the IDT staff training modules within Lakeland Care Inc.’s (LCI) learning management system.

Essential Competencies:

  • Demonstrates accountability for data-driven decision-making by independently managing and analyzing data to identify trends, gaps, and root causes. Will present findings in a clear, actionable format with recommended solutions.
  • Assist in the development, deployment, and maintenance of content and materials for reinforcement/refresher trainings and onboarding.
  • Assist in deployment of policy/procedure, processes, guidelines, and best practices. 
  • Leverage advanced tools and system capabilities, including AI, to streamline data analysis, enhance workflows, improve efficiency, and drive insight-informed decisions across responsibilities.
  • Work with all Program Operations departments to enhance consistency in formatting resources, language, and communications. 
  • Research, design, and implement adult learning strategies rooted in best practices, with a strong focus on engagement, real-world application, and sustained knowledge retention to ensure training is meaningful and impactful for staff.
  • Evaluate, monitor, and respond to practice trends to promote the delivery of high quality, effective care management. 
  • Use expertise to advance organizational practices and processes to align with larger strategic efforts. 
  • Exhibit a strong level of emotional intelligence in all day-to-day interactions. 
  • Support care management while keeping competencies current through activities such as, but not limited to leading IDT staff onboarding, electronic member record testing and application projects, and performing ‘fill-in’ tasks to support BP Department coverage needs.
  • At times, may assist with care management-related activities and projects to support organizational needs or goals.
  • Exhibit a strong level of emotional intelligence in all day-to-day interactions. 
  • Support care management while keeping competencies current through activities such as, but not limited to leading IDT staff onboarding, electronic member record testing and application projects, and performing ‘fill-in’ tasks to support BP Department coverage needs. 
  • At times, may assist with care management-related activities and projects to support organizational needs or goals.
  • Provide support to CM Supervisors and IDT staff in accordance with LCI policy in matters related to the care planning process. 
  • Partner effectively with other LCI departments in developing and reviewing resources, data, and trends for Care Management. 
  • Support mentoring, acclimating, and onboarding new care management employees to their role. 
  • Support any new lines of LCI business that may require CM resources.
  • Actively participate in and support LCI’s evolution of the Electronic Medical Record system; assist in testing and deployment of system changes and upgrades and provide clarification for practice. 
  • As directed, represent LCI to the community at large through professional interaction, clinical consultation, public speaking, media presentations, and participation in community advisory groups as requested.

Requirements

Education:

(Acceptable variations within the following criteria)

  • A four-year bachelor’s degree in adult education with one (1) year experience working with Family Care population.
  • A four-year bachelor’s degree in human services or related field or be a Certified Social Worker in the State of Wisconsin with one (1) year experience working with Family Care population, 
  • A four-year bachelor’s degree in any other area than human services with three (3) years’ experience working with Family Care population 
  • Or have a current RN License to practice in the State of Wisconsin, preferably with two (2) of skilled nursing experience in gerontology/disabilities and/or home care.

Additional Job Requirements:

  • Experience in Adult Learning/Education and training in design, development, and delivery.
  • Excellent computer, communication (verbal and written), and presentation skills.
  • Plan, prioritize, and organize work effectively to meet assigned timelines.
  • Leadership qualities necessary to support, mentor, and train staff and peers.
  • Ability to work from various work environments throughout the LCI service regions and state of Wisconsin as needed.
  • Ability to access members’ homes which are not required to comply with the ADA regulations.
  • Ability to lift up to 25 pounds.  
  • Current driver’s license, acceptable driving record, and proof of adequate insurance required.
