The Extender performs outreach, engagement, and follow-up with members while coordinating services and appointments. They support the Care Manager by assessing unmet health-related resource needs and participating in case conferences.
Applicants must be at least 18 years old with a high school diploma or equivalent. Two years of paid experience is required, including at least one year working directly with behavioral or mental health clients.

IIH Teams must be experienced/trained in TF-CBT, ESFT, and/or PCIT Evidenced Based Practices. Facility Based Lead Instructor: Must be a Qualified Professional (licensed professional preferred).
