Job Specific Duties:
Assists in the admission of new residents: interviews resident and/or family member on admission, performs assessments, orients new resident to the facility, assists in the formulation of the initial care plan
Reassesses residents at appropriate intervals – quarterly assessments as assigned. Communicates resident’s progress through recording pertinent notes and reporting to oncoming shift.
Observes the condition of the residents’ records any pertinent changes in the resident’s condition in the medical record and notifies the attending physician, responsible party, and nursing administration.
Provides a detailed report on each resident’s condition to the oncoming shift. Makes walking rounds at the beginning and end of each shift with off-going/oncoming nurse.
Obtains and properly records physician orders for all medications and treatments to be administered.
Takes vital signs as ordered by the physician/RN and record findings in the medical record.
Collects, labels, and arranges for transfer of specimens to lab as ordered by the attending physician. Notifies physician of abnormal results and carries out medical orders
Administers medications/performs wound care/treatments as ordered and documents on the eMAR/eTAR.
Follows policy and procedure for pharmacy services and facility policy and procedure. Checks expiration dates on medication. Keeps medication cart clean. Orders medications from pharmacy.
Maintains records of controlled substances. Accurately verifies count each shift.
Assists physicians with rounds and procedures.
Monitors resident’s intake/output and meal percentages.
Answers resident call lights and assist residents with ADLs as needed
Follows outlined procedures when resident’s vital signs have ceased: notifies supervisor, MD, family, and coroner if necessary. Prepares body for the mortician.
Notifies appropriate person when supplies are needed.
Determine duties which can be safely and appropriately delegated to unlicensed personnel.
Verify that Certified Nursing Assistants have reported to work, and collected assignment from the staffing form. Assign staff to cover sections if all nursing assistants have not reported to duty on time.
Supervises, instructs and provides direction to nursing assistants assigned to nursing team.
Provides instruction to nursing assistants regarding care of individual residents both orally and in writing using provided forms.
Checks on quantity and quality of work performed by nursing assistants. Immediately corrects and in private unacceptable performance or behavior by nursing assistants. Counsels and records on an Employee Counseling form any repeated unacceptable performance or behavior by a nursing assistant.
Reports unusual events, injuries or incidents concerning a resident, employee or visitor to the supervisor or Vice-President of Health Services. Initiates Incident Report form.
