The Nurse Practitioner conducts comprehensive health risk assessments for patients in residential or telehealth settings. They are responsible for diagnosing conditions, documenting visits per NCQA standards, and coordinating care with PCPs and caregivers.
Candidates must hold an active, unrestricted Nurse Practitioner license in Indiana and board certification from ANCC, AANP, or NCCPA. Additional requirements include BLS certification, Medicare/Medicaid enrollment, and a valid driver's license.

The Nurse Practitioner delivers annual risk assessment in a residential setting or telehealth, within the scope of practice for a Nurse Practitioner, as delegated by the Collaborative Physician.
