Lead Care Coordination Program Coordinator
Position Summary
The Lead Care Coordination Program Coordinator works in partnership with and under the direction of the Care Coordination Program Director to support the effective delivery of Care Coordination Wraparound services across all program locations. This position serves as a lead team member and key support to the Program Director by providing guidance, mentoring, training, and operational assistance to Care Coordinators while helping ensure quality service delivery, regulatory compliance, and program effectiveness.
The Lead Care Coordination Program Coordinator is not a supervisory position but serves as a subject matter resource, coach, and support to Care Coordinators and program staff. This role assists with onboarding, training, quality improvement initiatives, and the coordination of services to promote consistency and excellence throughout the program.
Essential Duties and Responsibilities
Program Leadership and Support
Support the Care Coordination Program Director in the daily operations and implementation of Care Coordination Wraparound services.
Serve as a resource, mentor, and coach to Care Coordinators across multiple program locations.
Assist with onboarding, training, and ongoing professional development of new and existing Care Coordinators.
Promote consistency in service delivery, documentation, and adherence to program standards and best practices.
Travel between program locations as needed to provide operational support, training, and program assistance.
Participate in team meetings, case reviews, and program planning activities.
Assist the Program Director with quality improvement initiatives and program development efforts.
Support community outreach activities and represent the agency at community events as needed.
Assist with preparation of weekly unit sheets and program reports.
Direct Service Responsibilities
Maintain a caseload of no more than fifteen (15) Care Coordination clients.
Provide case management services in accordance with Service Provision Level I or Level II requirements.
Ensure case management services are coordinated appropriately and do not duplicate services provided through institutional settings or discharge planning activities.
Process initial authorizations and related documentation as needed.
Facilitate coordination among service providers, families, schools, healthcare providers, and community partners to support positive client outcomes.
Compliance and Quality Assurance
Assist in maintaining compliance with network, COMAR, CSA, BHA, CARF, and other regulatory requirements.
Support preparation and ongoing maintenance of Care Coordination CARF accreditation activities.
Facilitate coordination of interagency program service delivery.
Help ensure the delivery of quality and appropriate services through adherence to agency policies, procedures, and regulatory standards.
Participate in case reviews and documentation audits to support program quality.
Identify opportunities for continuous quality improvement and assist in implementing corrective actions when needed.
Complete reports and documentation as required.
Other Duties
Perform other duties as assigned.
Minimum Qualifications
Bachelor's Degree preferred.
Experience working with children, youth, and families in behavioral health, human services, social services, or a related field preferred.
Energetic, dynamic, and passionate about working with children and families.
Excellent organizational, communication, and documentation skills.
Ability to build positive working relationships with staff, clients, families, community partners, and stakeholders.
Ability to observe, assess, and accurately document the functioning of children and families.
Ability to manage multiple priorities and work effectively in a fast-paced environment.
Ability to handle stressful situations with professionalism and sound judgment.
Must meet Care Coordinator training and certification requirements established by DHMH.
Must complete trainings required by CSA and BHA, including CASII, ESCII, and CANS.
Must complete Wraparound Certification.
Valid driver's license and reliable transportation required for travel between program locations.
Compensation
Salary: $55,000 - $60,000 annually
