Clinician, Case Management - Psychogeriatric

The Case Management Clinician works one on one or in groups with individuals who choose our program to meet their needs. They will provide support to individuals 65+ as they improve their health and quality of life in the face of Mental Health and/or Addictions concerns and the accompanying barriers they experience. The Seniors Outreach and Recovery (SOAR) Case Management Clinician assesses the person’s needs, strengths, and supports to co-develop goals and a customized recovery and support plan. You would work from a trauma informed perspective as part of a team and part of a community to advocate, challenge stigma, and prevent harm.

The Case Management Clinician acts as a special resource in psychogeriatric care, including co- occurring disorders; provides consultation and formal teaching; is responsible for the provision of clinical services to the psychogeriatric client through assessment, treatment, monitoring and directing care; act as liaison with community agencies and care providers; and assists in community development activities.

WHAT WE'RE OFFERING:

  • What you'll be making - Starting rate for this role is $30.01 per hour
    • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($37.26 per hour)
    • For internal applicants, grid level placement will occur as per the Collective Agreement
  • When you'll work - 70 hours biweekly; Monday to Friday, 8:30am-4:30pm
  • Your pension plan –Immediately enrolled in the Healthcare of Ontario Pension Plan (HOOPP)
  • What your benefits will be –After a 90-day waiting period, will be enrolled in extended healthcare benefits

HOW YOU WILL BE MAKING A DIFFERENCE:

  • Manage a caseload of clients involved in the program
  • Assess, coordinate and provide clinical care for older adults that both facilitates and supports recovery for those experiencing a broad range of mental health challenges including dementia as well as clinical interventions for care partners of individuals living with dementia and other mental disorders
  • Provides psychosocial interventions in individual and group therapy
  • Provides psycho-education around meds and symptom
  • Crisis prevention and intervention, and safety plan
  • Monitor symptomology, medication, and activities of daily living to help improve individuals’ ability to function on a day to day basis and within the community
  • Assist with resource navigation and linkage with community mental health services to support transition and ongoing care
  • Ongoing collaboration and communication with the inter-professional team and with the client’s social and community networks
  • Employs technological approaches such as video-conferencing, telephone and other methods to deliver clinical and educational programming
  • Act as a liaison and client advocate among those involved with the client’s care including family, other care-givers, medical and other multi-disciplinary providers
  • Provide skill development, linkages, resources, information, counselling and advocacy
  • Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles

WHAT YOU'LL NEED TO APPLY:

Police Vulnerable Sector Check Requirements
Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed within the last six months. In order to avoid delays in beginning a position, interested applicants who are in need of a PVSC are encouraged to apply for one as soon as possible.

We require the following qualifications:

  • RPN, RN, Bachelor's or Master's Degree in Occupational Therapy, Nursing, Social Work, Psychology or related field (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
  • Currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers, the College of Nurses of Ontario, the College of Registered Psychotherapists of Ontario, or the College of Occupational Therapists of Ontario
  • Valid G driver’s licence, access a vehicle for personal transportation
  • Transportation of clients in personal vehicle required; two million-dollar ($2 000 000.00) liability insurance required
  • Current, satisfactory Police Vulnerable Sector Check (PVSC)
  • Demonstrated ability to attend work regularly

We prefer the following qualifications:

  • Current First Aid/CPR certification, ASIST training, and Non-Violent Crisis Intervention training
  • Experience working with seniors and detailed knowledge of their needs
  • Experience and detailed knowledge of serious mental illnesses usually gained by several years of working with this population
  • Experience in working in a community-based setting with an extensive knowledge of social assistance programs and both generic and specialized local social support networks is essential
  • Experience in crisis intervention/management and crisis resolution
  • Experience working with individuals with an ABI or Development Disability
