Conduct remote patient assessments and monitor health status for behavioral health patients. Coordinate care and document patient interactions accurately within multiple computer systems.
Requires a current RN license in specific states or a compact license and a bachelor's degree. Must have at least 3 years of experience in behavioral clinical settings and care management.
