The RN Case Manager performs patient assessments and collaborates with a care team to implement personalized plans of care. They are responsible for supervising LPNs and HHAs while providing patient education and timely clinical documentation.
Candidates must have a current RN license and CPR certification, along with a valid driver's license and reliable transportation. At least one year of clinical experience is preferred, though candidates with less may be eligible via an intern program or VP approval.
Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
