Responsibilities

The Social Worker is responsible for developing comprehensive psycho-social histories and individual service plans for medically fragile children. They provide community support through home visits, school visits, and court hearings to ensure tailored care for each client.

Requirements

Candidates must hold a Master's degree in Social Work and a current licensure in the State of Maryland. Additionally, First Aid/CPR certification and at least one year of supervised social work experience in a healthcare setting are required.

JOB DUTIES

This position requires full understanding and active participation in fulfilling the mission of Second Family, Inc.  It is expected that the employee demonstrates behavior consistent with core values as well as follows all legal requirement in implementing duties according to policies and procedures.  The Social Worker reports to the Director of Social Work and is responsible for collaboratively developing a Psycho-Social history for each child. This history will include detailed information about the child's previous family background, medical history, placement history prior to their arrival at Second Family, Inc., services provided during their time with Second Family, Inc., and future plans tailored to their individual needs.

Responsibilities 

  • Evaluates each child to develop their Individual Service Plans (ISP) and Individual Educational Plans (IEP)
  • Develops Behavior Plans for clients on Psychotropic medications and/or based on other behavioral issues
  • Provides community support for the children involving home visits, school visits, court hearings/reviews
  • Will be required to work at any facility owned by Second Family, Inc. and is responsible for own transportation. 

 Requirements  

  • Master’s degree in social work from an accredited School of Social Work 
  • Current licensure in the State of Maryland will be required
  • First Aid and CPR certified by American Heart Association
  • A minimum of one year supervised social work experience in a health care setting, working directly with individuals.
