ABOUT THE ROLE
This is a dual role as Community Health Work (CHW) is embedded into all our diabetes prevention and management work.
As a Community Health Worker (CHW), you will promote health in the community and with individual members in a culturally competent and responsive way. You will act as a liaison and advocate to improve relationships between historically marginalized groups, service institutions, and the YMCA of WNC through education and referrals. You will connect community members to services specific to their needs, act as a liaison with community organizations, and assist in creating community outreach and health-related education activities. You will provide support to the Food Access Team of the YMCA of WNC in various ways including mobile markets, processing food at the Nutrition Hub, etc.
As a Certified National Diabetes Prevention Program (NDPP) Lifestyle Coach, you will screen and educate individuals at outreach events, facilitate group sessions using the CDC-approved curriculum, drive behavior change per the goals of the NDPP, provide accountability by tracking and reviewing class documents and participant logs, foster group support by creating a safe and encouraging environment that builds peer support and collaborative problem-solving to overcome lifestyle barriers, maintain program fidelity by delivering high-quality group facilitation and coaching and maintain excellent record keeping in compliance with HIPAA regulations.
ESSENTIAL FUNCTIONS
Supports service line/department in actively working toward association-level objectives, as measured by identified goals/Key Performance Indicators
Advises communities on issues related to general health (e.g., nutrition, physical activity, stress management, etc.) and chronic condition prevention and management (e.g., diabetes education)
Increases health knowledge and self-sufficiency of community members through various activities such as outreach, community education, social support, and advocacy
Educates individuals and communities on preventing and managing health conditions, supports developing healthy behaviors, and advocates for social change
Provides input for marketing and educational materials using various methods and media (e.g., fliers, blogs, social media posts)
Promotes YMCA of WNC programs within the community and supports participants in YMCA of WNC Evidence-Based Health Intervention (EBHI) programs
Receives, communicates, and responds to referrals from fellow organizations and care agencies and connects community members to external support
Builds relationships with health and resource organizations for referral and collaboration
May deliver preventative health services and/or collect data for community health assets and needs assessments
Researches developing community/public health issues; works with Community Health leadership to identify our role in addressing these concerns locally
Performs all other duties as assigned
ABOUT YOU
Qualifications
Minimum of 2 years of professional experience in public or community health and/or community engagement
Community Health Worker certification or ability to obtain within 6 months of hire
National DPP Lifestyle Coach certification or ability to obtain within 6 months of hire
Associate degree (or higher) in a health-related field is preferred.
Direct experience in community health or addressing health disparities is preferred.
Bi-lingual in Spanish and English preferred.
Knowledge, Skills, and Abilities
Strong understanding of participants’ cultures, values, and health practices/beliefs
Strong organizational skills and ability to prioritize workloads in order to meet deadlines
Detail-oriented and conscientious
Excellent interpersonal, public relations, and communications skills
Comfortability in both office and community-based work environments
Efficient personal computer skills and experience with standard business software
