Washington County Mental Health Services is seeking Integrated Care Navigators to help clients and providers navigate administrative, referral, and follow-up systems. Utilizing the EMR database, Navigators manage records requests, complete service referrals, track appointments, and obtain required Releases of Information. The role also supports outreach, missed appointment procedures, and Medicaid or benefits application assistance to ensure seamless care.
Integrated Care Navigators connect clients with essential primary care, specialty providers, and community resources like housing and benefits. By resolving barriers to care, standardizing records tracking, and speeding up closed-loop referrals, Navigators ensure clients stay connected to vital health services and receive well-coordinated community support.
Summary of Duties
Complete, track, and follow up on client referrals for health, housing, crisis, employment, and specialty services in the EMR
Link clients with primary care providers, monitor appointment attendance, request medical records, and build relationships with local medical offices
Review client charts, obtain required Releases of Information (ROIs), and maintain accurate care coordination notes in the agency EMR
Identify clients eligible for Medicaid, Medicare, SNAP, or other benefits and assist them with the application process
Execute outreach and missed appointment protocols, track care pathway adherence, and problem-solve access barriers with clients and providers
Participate in staff huddles, coordinate care across interdisciplinary teams, and communicate with external partners to support service delivery
Assist with intake activities as assigned, including preparing client charts, routing information, recording health history, and collecting basic vital signs
Compensation and Benefits

The starting pay range for this position is $28.67 - $30.13 per hour.
QualificationsSummary of Qualifications
Education: High school diploma required; Associate degree (or equivalent relevant experience) preferred
Experience in care coordination, case management, medical/behavioral health, or community-based services preferred; familiarity with Medicaid and community referrals is highly desirable
Proficiency with EMR systems, Microsoft Excel, Word, Outlook, data entry, and record management workflows
Strong verbal and written communication skills to build professional relationships with clients, interdisciplinary care teams, primary care offices, and community partners
Practical problem-solving skills to address service linkage barriers, application processes, and care follow-up using a client-centered, trauma-informed approach
Workplace Conditions
Work takes place in a multiple setting. Frequent transportation of clients in one's own vehicle. This role requires some lifting up to 50lbs and ability to apply restraints to clients within agency training and guidance.
