JOB SUMMARY

Reporting to the Resident Care Manager, the Licensed Practical Nurse (LPN) ensures the general well-being of residents by providing a system of services for residents in a manner and environment that provides dignity and full recognition of each resident’s individuality. The Licensed Practical Nurse will manage and supervise the delivery of services by certified staff. The Licensed Practical Nurse respectfully interacts with all residents, family members, visitors, colleagues and staff. This is a safety sensitive position.

ESSENTIAL FUNCTIONS

Coordinates admission and orients new residents to the facility, ensuring satisfactory integration
Monitors and maintains records reflecting health and mental status of residents and reports any changes to the resident’s physician in a timely manner
Administers treatment and records resident’s response. Makes recommendations regarding medication to physician and works with pharmacy to assist with the appropriate handling, dispensing and destruction of medication
Ensures that all resident rights listed in the Resident Bill of Rights are honored and respected by all staff members
Executes infection control duties
Coordinates with dietary staff to ensure nutritional and therapeutic diets are served to residents
Investigates and documents any resident incident or issue
Coordinates resident discharges and transfers
Observes and may assist in direct resident care
Assists new staff and with ongoing training
Attends meetings and trainings as assigned
Regular and reliable attendance is an essential function of this job

WORKING CONDITIONS

Work is conducted primarily in health care facility. There is possible exposure to communicable diseases and blood and body fluids that may contain HIV and/or HBV. This position frequently requires working within strict deadlines, under stressful conditions and with a variety of personalities, who may at times be emotional.

Licensed Florida LPN; required to maintain licensure
Current CPR certification required
Completed LPN Supervisory Course or ability to complete within 6 months of employment
Two or more years of related experience and/or training in geriatrics in long term care, home health or other health care setting
Excellent verbal and written communication skills
Computer literacy which includes, but is not limited to, Microsoft Office suite
