Behavioral Health Intake Specialist LPCC/LISW
A Great Opportunity
Full/Part Time Monday - Friday 9am-5pm or afternoons and evenings when needed
$65,000-$75,000 based on education and experience plus monthly incentive program for exceeding productivity expectations
Responsibilities may include:
- Provide clinical oversight
- Intake assessments, conduct behavioral health intakes and psychosocial assessments
- Team resource support within a regional BHI team serving skilled nursing and long-term care (SNF/LTC) settings.
- Support Value-Based Care initiatives, and serve as a clinical
- Resource escalation point for Behavioral Health Coordinators
- Ideal for clinicians who enjoy systems-level impact,
- Interdisciplinary collaboration, and shaping a growing behavioral health program.
Requirements for this position include:
- At least 18 years of age
- must be licensed as an LPCC, LPCC-S, LISW, LISW-S or IMFT
- Master’s degree in social work, professional counseling or marriage and family therapy;
- license must be maintained during employment.
- Clinical experience treating individuals with a variety of diagnoses and presenting issues is preferred.
- Must be Medicare-eligible to provide intake and assessment services and possess a minimum of three (3) years of behavioral health, case management, or related clinical experience.
- Experience working in skilled nursing facilities, long-term care settings, geriatric populations, Value-Based Care programs, and/or Behavioral Health Integration (BHI) programs is preferred.
- Experience conducting diagnostic assessments and behavioral health intake preferred. Prior team lead, supervisory, or clinical leadership experience is also preferred.
- Must be responsible, mature and flexible; must possess excellent decision-making, time management and communication skills
- Must be able to respond calmly to crisis situations; demonstrate the ability to direct others in a confident, respectful manner and exercise good judgment in interactions with residents, families, employees, peers and regulatory agency personnel.
- Must appropriately handle confidential information.
- Must have a valid driver's license and reliable transportation.
- Bilingual language skills are a plus.
