Join a compassionate and supportive hospice team dedicated to making a meaningful difference in the lives of patients and families. The Care Team Hospice is seeking a Social Worker (MSW) to provide emotional support, counseling, advocacy, and community resource coordination for hospice patients and their loved ones while collaborating closely with the interdisciplinary team to ensure compassionate, patient-centered end-of-life care.

Schedule, Hours & Territory
• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Reading, PA, and up to a 1-hour surrounding radius
• Participation in an on-call rotation may be required, with additional compensation provided

Key Responsibilities

• Conduct psychosocial assessments
• Provide counseling and emotional support to patients and families
• Assist with advanced care planning and coordination of community resources
• Participate in interdisciplinary team meetings
• Document services accurately and timely in the EMR HCHB (Homecare Homebase)
Qualifications

• Master's Degree in Social Work (MSW) from an accredited program preferred.

• Bachelor's Degree in Social Work (BSW) accepted with required supervision in accordance with state regulations.

• Previous experience in hospice, home health, healthcare, or case management preferred.

• Knowledge of psychosocial assessment, crisis intervention, grief counseling, and community resources.

Field Role Requirements

• Valid driver's license, reliable transportation, and current automobile insurance.

• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.

• Current TB testing in accordance with agency/State policy.

• Current CPR certification as required for the role.
