Physical Therapist Responsibilities
As a Home Health Physical Therapist, you will:
- Evaluate patients in their home environment
- Develop and implement individualized physical therapy treatment plans
- Provide skilled physical therapy interventions based on patient needs
- Collaborate with physicians, nurses, therapists, and other members of the care team
- Monitor patient progress and adjust treatment plans as appropriate
- Educate patients and caregivers on treatment plans, exercises, and strategies to support recovery
- Maintain timely and accurate clinical documentation
- Manage your daily schedule and patient visits independently
Qualifications
We’re looking for compassionate, organized, and patient-focused Physical Therapists who have:
- Bachelor’s or Master’s degree in Physical Therapy from an accredited college or university
- Current Physical Therapist (PT) license in the state where you will practice
- Minimum of one year of healthcare experience
- Current valid driver’s license and reliable ability to travel between patient homes
- Ability to spend approximately 20% of the workday driving to and from patient locations
- Strong time-management and organizational skills
- Ability to consistently meet critical documentation and charting deadlines
- Comfort using tablets, smartphones, and EMR/EHR software
- Ability to work independently while collaborating effectively with a multidisciplinary care team
- Compassionate approach to patient care and a commitment to improving patient outcomes
Schedule & Travel
Schedule: Monday–Friday, 8:00 AM–6:00 PM
Weekend: 1 weekend per month
Holidays: 2 holidays per year
Travel: Local travel to patients' residential homes
