Your Role
Working under the supervision of a licensed Physical Therapist, you will:
Provide individualized physical therapy treatments in patients' homes
Implement treatment plans focused on improving mobility, strength, balance, endurance, and independence
Monitor progress and communicate changes with the supervising Physical Therapist
Educate patients and caregivers on home exercise programs and fall prevention
Recommend assistive devices and home safety modifications
Complete accurate documentation in accordance with Medicare and agency guidelines
Collaborate with physicians, nurses, therapists, and other healthcare professionals to deliver outstanding patient care
Qualifications
Required
Graduate of an accredited Physical Therapist Assistant program
Current Arizona PTA license
CPR Certification (or ability to obtain)
Valid Arizona driver's license
Reliable transportation and current auto insurance
Home health experience is preferred but not required.

QualificationsEducation and Experience: Must be a graduate from an accredited Physical Therapy program and licensed by the state of Arizona.
