Job DetailsHIV Non-Medical Case Manager

The Non-Medical HIV Case Manager provides strengths-based case management services to individuals living with HIV/AIDS. This role assists clients in accessing medical care, support services, insurance coverage, housing resources, transportation, and other community resources that promote health and well-being.

Responsibilities

Conduct comprehensive client assessments and develop individualized service plans.
Assist clients in accessing HIV medical care and support services.
Provide referrals and linkage to community resources, including housing, food assistance, transportation, mental health services, and benefits programs.
Monitor client progress and maintain ongoing contact to support retention in care.
Advocate for clients and help address barriers to treatment and services.
Maintain accurate and timely documentation in accordance with program requirements.
Participate in interdisciplinary team meetings and case conferences.
Ensure compliance with Ryan White, grant, and organizational requirements.
Maintain client confidentiality and adhere to HIPAA regulations.

QualificationsRequirements

Bachelor’s degree required, master’s degree preferred.

Experience in case management, preferably with individuals living with HIV/AIDS.
Knowledge of HIV care, treatment, and available community resources.
Strong interpersonal, communication, and advocacy skills.
Ability to work independently and as part of an interdisciplinary team.
Excellent organizational and time-management skills.
Sensitivity to the unique challenges faced by individuals living with HIV.
