About this role

This role provides biopsychosocial assessments, care coordination, discharge planning, and protective interventions for vulnerable adults. The LMSW will support APS-related cases, risk assessments, legal matters (guardianship/POA), and connect patients with essential community and medical resources.

Key Responsibilities:

  • Perform biopsychosocial assessments and initial evaluations
  • Address safety risks, abuse/neglect concerns, guardianship, and POA
  • Coordinate discharge planning, DME, rehab, and long-term care referrals
  • Collaborate with care teams in ED, ICU, Med-Surg, L&D, and NICU
  • Document assessments, plans, and interventions per standards
  • Provide education on Advance Directives, ADA, and community resources
  • Navigate Medicaid, Medicare, transportation, and medication assistance programs


Requirements

  • Master’s degree in Social Work (MSW)
  • Washington State LMSW license (or eligibility)
  • Experience in acute care, APS, crisis intervention, or case management preferred
