About this role
This role provides biopsychosocial assessments, care coordination, discharge planning, and protective interventions for vulnerable adults. The LMSW will support APS-related cases, risk assessments, legal matters (guardianship/POA), and connect patients with essential community and medical resources.
Key Responsibilities:
- Perform biopsychosocial assessments and initial evaluations
- Address safety risks, abuse/neglect concerns, guardianship, and POA
- Coordinate discharge planning, DME, rehab, and long-term care referrals
- Collaborate with care teams in ED, ICU, Med-Surg, L&D, and NICU
- Document assessments, plans, and interventions per standards
- Provide education on Advance Directives, ADA, and community resources
- Navigate Medicaid, Medicare, transportation, and medication assistance programs
