The G. Wayne Fralin Free Clinic provides compassionate healthcare to people experiencing homelessness and others in crisis through services provided within the Clinic, Rescue Mission programs, Respite Care, and Medical Street Outreach.

Purpose

The Clinic Coordinator / Registered Nurse serves as a clinical leader and key member of the Fralin Free Clinic team, coordinating the daily flow of patient care while providing direct nursing services within the scope of RN practice.

Working under the leadership of the Clinic Manager and in collaboration with the Volunteer Medical Director, paid and volunteer medical providers, Medical Assistants, and other Rescue Mission teams, the Clinic Coordinator / RN helps ensure that patients receive safe, compassionate, coordinated, and timely care. The position provides clinical direction and coordination during patient care while supporting the Clinic Manager's overall responsibility for team leadership, personnel supervision, and Clinic operations.

The position provides clinical coordination and nursing support wherever Fralin Clinic care is delivered, including within the Clinic, in Rescue Mission Respite Care, and through Medical Street Outreach. The Clinic Coordinator / RN helps translate provider direction and established Clinic systems into effective daily patient care, supports paid and volunteer providers, coordinates clinical follow-up, and identifies and communicates concerns requiring additional clinical or management attention.

Qualifications

Evidence of a positive Christian witness and lifestyle consistent with the mission of the Rescue Mission of Roanoke.

Current Registered Nurse licensure in the Commonwealth of Virginia, or eligibility for appropriate Virginia licensure.

Demonstrated compassion, patience, sound judgment, and ability to work effectively with individuals experiencing homelessness, behavioral health challenges, substance use disorders, and other complex needs.

Strong clinical assessment, critical-thinking, organizational, and decision-making skills.

Ability to prioritize and coordinate multiple patient-care needs in a fast-paced environment.

Strong oral and written communication skills.

Ability to maintain accurate and timely electronic medical records and other clinical documentation.

Working knowledge of HIPAA, OSHA, and applicable healthcare clinic guidelines and regulations.

Ability to work collaboratively with paid and volunteer healthcare professionals, community agencies, Rescue Mission programs, and other partners.

Understanding of volunteer engagement and demonstrated ability to welcome, support, utilize, and appreciate volunteers as integral members of the care team.

Ability to communicate the work of the Rescue Mission and Fralin Free Clinic to community groups as coordinated with Mission leadership.

Areas of Responsibility

1. Clinical Coordination & Patient Care

Coordinate the daily flow of clinical services, helping ensure patients are appropriately triaged, providers are supported, orders are completed, follow-up needs are identified, and patient care moves efficiently through the Clinic.

Triage and assess patients seeking Medical, Dental, Mental Health, Optometry, and other Fralin Clinic services within the scope of RN practice.

Provide direct nursing care, patient education, assessment, testing, treatments, medication support, and other clinical services within RN scope of practice and according to provider direction and established Clinic protocols.

Proactively assist paid and volunteer medical providers with examinations, treatments, procedures, and other patient-care needs.

Identify changes in patient condition, clinical concerns, or other needs requiring provider involvement and communicate them promptly to the appropriate medical professional.

Help ensure provider orders are understood, implemented, documented, and appropriately followed through.

Coordinate with Medical Assistants and other Clinic team members to ensure examination rooms, equipment, medications, supplies, and other resources are ready to support patient care.

Promote a patient-centered environment characterized by compassion, dignity, respect, safety, and appropriate professional boundaries.

2. Respite Care & Continuity of Care

Provide clinical coordination and nursing oversight for patients staying in Rescue Mission Respite Care.

Proactively and routinely visit Respite areas and patients rather than relying solely on patients to return to the Clinic for follow-up.

Assess patient status and needs, reinforce provider instructions and care plans, and identify changes or concerns requiring provider attention.

Coordinate with Medical Assistants and other Clinic team members to ensure consistent follow-up with Respite patients.

Support patients in following through with medications, testing, appointments, referrals, discharge instructions, and other recommended care.

Ensure relevant Respite encounters, assessments, interventions, and follow-up are appropriately documented in the medical record.

Communicate with Shelter and other Rescue Mission team members as appropriate to support patient safety and continuity of care while maintaining appropriate confidentiality.

Coordinate Respite admissions, ongoing clinical needs, and discharge planning within established Clinic and Rescue Mission procedures.

3. Medical Street Outreach

Participate in and help clinically coordinate Fralin Clinic Medical Street Outreach activities as scheduled.

Provide nursing assessment and care within RN scope of practice in outreach settings.

Support paid and volunteer medical providers participating in Street Outreach.

Help ensure appropriate medications, medical supplies, equipment, documentation resources, and other clinical materials are available for outreach.

Ensure care provided through Street Outreach is appropriately documented and integrated into the patient's Fralin Clinic medical record when applicable.

Identify follow-up needs and help connect individuals encountered through Street Outreach with Fralin Clinic, Rescue Mission services, and appropriate community healthcare resources.

Help create continuity between care provided on the street and ongoing medical care and services.

4. Provider & Volunteer Clinical Support

Help create a Clinic environment in which paid and volunteer providers are well-supported and able to use their skills effectively in caring for patients.

Welcome and clinically orient new medical volunteers to Clinic workflows, patient-care procedures, documentation expectations, safety practices, and available resources.

Proactively support volunteer providers during Clinic sessions, helping ensure they have the information, assistance, supplies, and resources necessary to provide excellent patient care.

Coordinate with the Clinic Manager, Medical Assistant, and Volunteer Services regarding volunteer needs, assignments, schedules, orientation, and ongoing engagement.

Communicate clinical expectations and workflow needs to volunteers and provide appropriate guidance during their service.

Identify opportunities to better utilize volunteer expertise and communicate needs, gaps, or concerns to the Clinic Manager.

5. Medical Records, Provider Orders & Clinical Follow-Up

Ensure timely, accurate, and complete clinical documentation in the electronic medical record.

Review patient records and provider orders to help ensure laboratory tests, medications, referrals, treatments, procedures, and other ordered services are completed and appropriately documented.

Coordinate laboratory orders, specimen collection, urine drug screening, and other testing as ordered.

Help ensure procedures and services performed during patient encounters are accurately reflected in the EMR.

Monitor outstanding clinical follow-up needs and coordinate with providers, Medical Assistants, patients, and community partners to promote completion.

Communicate documentation or workflow concerns to the Clinic Manager and help implement corrective processes.

6. Medications & Clinical Supplies

Administer, receive, store, document, and distribute medications within RN scope of practice, provider direction, and established Clinic procedures.

Support appropriate management of prescription and over-the-counter medications provided through Fralin Free Clinic.

Help maintain required medication records and logs.

Monitor clinical supplies, medications, and equipment and communicate anticipated needs to the Clinic Manager.

Help maintain organized, safe, and appropriately stocked clinical and medication areas.

7. Medical Referrals & Community Coordination

Communicate with hospitals, healthcare facilities, discharge planners, medical practices, and other referring organizations regarding patients being referred to Rescue Mission services.

Review available medical information and provide appropriate clinical input regarding the individual's needs and the Rescue Mission's ability to safely support those needs.

Help ensure referrals from medical facilities to Rescue Mission Emergency Shelters and Respite Care are clinically appropriate and consistent with established admission criteria and available resources.

Elevate complex or uncertain referrals to the appropriate medical provider, Clinic Manager, or Rescue Mission leader rather than making decisions outside established authority or scope.

Coordinate with Shelter, Care Team, Recovery, and other Rescue Mission programs to support continuity of care.

Maintain positive and collaborative relationships with community healthcare providers and agencies on behalf of Fralin Free Clinic.

8. Team Communication, Quality & Safety

Keep the Clinic Manager informed of significant clinical, operational, patient-care, volunteer, or administrative issues.

Provide clinical direction and coordination to Medical Assistants and other appropriate Clinic team members during patient care, while personnel supervision and performance management remain the responsibility of the Clinic Manager.

Participate in quality improvement activities and help identify opportunities to strengthen patient care, documentation, workflow, safety, and follow-up.

Maintain awareness of safety and security concerns and respond according to established Rescue Mission procedures.

Participate in appropriate conflict prevention and de-escalation efforts.

Maintain HIPAA and confidentiality standards in all patient interactions and communications.

Participate in Rescue Mission meetings, training, continuing education, events, and other activities as requested.

9. Volunteer Engagement

Volunteer engagement is an essential strategy for expanding the capacity and impact of Fralin Free Clinic. The Clinic Coordinator / RN will model a culture in which volunteers are welcomed, appreciated, appropriately utilized, and recognized as essential partners in providing excellent patient care.

The Clinic Coordinator / RN will work collaboratively with the Clinic Manager, Medical Assistant, Volunteer Services, and other team members to ensure volunteer healthcare professionals have meaningful opportunities to use their skills while receiving the orientation, clinical support, communication, and resources necessary to serve effectively.

10. Other Responsibilities

All Rescue Mission of Roanoke team members may be asked to complete other duties as assigned by leadership/management. The expectation is to complete these assignments as requested.
