Monday - Friday, 12:00PM - 8:00PM OR Tuesday - Friday 12:00PM - 8:00PM and Saturday 9:00AM - 5:00PM
Job Summary
Pilsen Wellness Center is seeking a dedicated and compassionate Case Manager to participate in the implementation of the individual’s service plan, specifically designed to increase an individual’s skills in independence in daily living, economic self-sufficiency and community integration.
Responsibilities
- Assists the client in achieving independence (e.g., housing, finances)
- Assists with supported employment/education as needed
- Maintain timely and accurate clinical documentation
- Helps participants to access the services specified in their service plans
- Adhere to ethical and confidentiality standards
Qualifications
- High School Diploma or GED
- Active Drivers License
- Bilingual (English/Spanish) preferred
- 3-5 years of administrative and clerical assistance experience
- Cultural humility and the ability to build trust with diverse populations
- Strong documentation and collaboration skills
Compensation & Benefits
