What You'll Do
- Provide physical therapy services and patient education in the home in accordance with the physician’s Plan of Care
- Complete visits and documentation accurately and within established timelines and agency standards
- Monitor patient progress and promptly report changes in condition or schedule to the supervising therapist or nurse
- Collaborate with the interdisciplinary care team, including participation in care conferences and referrals as appropriate
- Follow all safety, infection control, and equipment-use protocols to ensure a safe environment for patients and staff
- Maintain strict patient confidentiality and protect sensitive information at all times
- Comply with all agency policies, as well as state, federal, and professional standards
- Participate in ongoing training, staff meetings, and quality improvement activities
- Perform additional duties as assigned
What You Bring
- Current license to practice as a Physical Therapist Assistant in the state(s) of employment.
- Graduate of an accredited physical therapist assistant program approved by the American Physical Therapy Association.
- Minimum of six (6) months experience as a Physical Therapist Assistant.
- Demonstrated written, verbal communication skills, and interpersonal skills.
- Licensed driver with automobile insured in accordance with state/Agency requirements. Reliable transportation with good driving record.
