Home Health Care Social Worker Responsibilities & Duties
- Provide emotional and practical support to patients and their families in their homes
- Assess patients' needs and develop individualized care plans
- Link patients with community resources and services
- Collaborate with interdisciplinary healthcare teams to provide holistic care
- Maintain accurate and up-to-date patient records
- Stay current with advancements in home health care and social work practices
Home Health Care Social Worker Qualifications & Skills
- Master's degree in Social Work (MSW)
- Licensed Clinical Social Worker (LCSW) certification
- Experience in health care or home health care settings
- Strong understanding of health care systems and policies
- Proficiency in electronic health records (EHR) systems
- Fluency in multiple languages
- Advanced training in geriatric care or chronic illness management
- Excellent interpersonal and communication skills
- Ability to work independently and manage a caseload
- Bachelor's or Master's degree in Social Work
- Licensed in good standing as a social worker
- 2+ years of experience in home health care or related field
- Excellent communication and interpersonal skills
- Strong organizational and problem-solving skills
- A commitment to patient-centered care and advocacy
- Previous Home health experience that works with Medicare, Medicaid, and/or VA patients
