Job DetailsJob Location: Albany-Academy Main-PCFC - Albany, NY 12208Position Type: Full TimeSalary Range: $24.00 - $26.00 HourlyTravel Percentage: Up to 50% As a Case Manager, you will support clients in the Residential System and perform duties in collaboration with a clinician. You will report to the Case Management Supervisor, Residential and you will work onsite with community-based travel.

Responsibilities

Arrange and attend home visits
Prepare transition plans

Coordinate Independent Living Services

Be the primary contact between Schools and Residential
Coach family visits
Complete various assessments, including FASP and SPR
Complete accurate case notes and input the notes into Connections database
Provide transportation for clients
Maintain contact with DSS and family
Function as a contributing member of the interdisciplinary treatment team
Attend dorm meetings
Attend treatment review meetings

Requirements and Education

CM1: Requires an HS Diploma/GED and two years of experience in social services or case management
CM2: Bachelor’s Degree and two years of experience in social services, HS Diploma and three years of social services or case management experience
Lead CM: Bachelor’s degree and three years of experience in case management, or HS and four years of experience in case management
CM Supervisor: Bachelor’s degree and four years of experience in case management, or HS and 6-8 years of experience in case management
Experience with individuals in care systems preferred.
Must pass physical.
