The Role

Milu Health is seeking a clinically experienced and detail-oriented Medication Review and Medication Therapy Management Pharmacist to join our clinical team. This remote, part-time role is ideal for a pharmacist who thrives in a data-rich, patient-focused environment and wants to be part of a forward-thinking team shaping value-based pharmacy interventions.

You will provide expert review of member medication profiles, assess opportunities for medication optimization, adherence improvement, and clinical impact, and support our navigation and engagement strategies through personalized recommendations.


Key Responsibilities

  • Conduct comprehensive medication reviews (CMRs) and targeted medication reviews (TMRs) using clinical and claims data

  • Identify drug therapy problems, duplications, adherence issues, high-cost drugs with therapeutic alternatives, and clinical gaps

  • Communicate medication recommendations to members, providers, and care navigators

  • Collaborate with Milu's clinical and data teams to refine MTM algorithms and protocols for identifying actionable pharmacy interventions

  • Maintain accurate documentation of all reviews, recommendations, and follow-up outcomes in internal systems

  • Stay up to date with clinical guidelines, drug formulary trends, and biosimilar/generic equivalency opportunities

Qualifications

  • Doctor of Pharmacy (PharmD) required

  • Active, unrestricted pharmacist license in at least one U.S. state (multiple states preferred)

  • Minimum 3 years of experience in MTM, clinical pharmacy, managed care, or pharmacy benefits

  • Strong understanding of pharmacotherapy, formulary management, and cost-containment strategies

  • Familiarity with EHR or claims-based pharmacy data and population health tools

  • Excellent written and verbal communication skills

  • Comfortable working in a fast-paced, startup environment and independently managing clinical workload
