Amplar Health Home Hospital is continuing to expand its Transition Care Service creating opportunities for Registered Nurses to join the team. Whether you're currently working in acute care and looking for a change or working in another nursing setting and wanting to broaden your experience, TCS offers the opportunity to apply your clinical knowledge in a rewarding sub-acute environment, supporting patients as they recover or improve their health outcomes. We are seeking compassionate, motivated RNs who are committed to person-centred care and making a genuine difference in patients' recovery journeys.
Permanent opportunities are available on a full-time or part-time basis (minimum 0.6 FTE), based either in Adelaide’s CBD or at Walkerville.
Your opportunity
This opportunity will provide you a chance to exercise your experience as a Registered Nurse, providing comprehensive care to patients with a broad range of needs in a new dynamic setting outside of the hospital, in addition to:
- Delivery high quality patient care across diverse clinical presentations
- Coordination and delivery of high-quality patient centred care within a multidisciplinary environment
- Work in a professionally simulating, rewarding environment that utilises innovative technology
- Maintain accurate patient records
- Respond confidently to clinical changes and other emergency situations
- Mentor junior staff and contribute to continuous improvement
- Participate in ongoing professional development and training
About you
You'll enjoy a balanced 7-day rotating roster (mornings, afternoon and nights) offering variety, flexibility, and work-life balance, within a supportive team environment.
- At least 1 years’ experience as a Registered Nurse
- Current unrestricted AHPRA registration
- Hold current First Aid and CPR certificates
- Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
- If successful, willing and able to provide proof of up-to-date vaccinations.
- Nurses with acute care experience are encouraged to apply.
