Join a team dedicated to supporting the community. As a Social Worker Coordinator, you will play an important role in assisting social workers and mental health professionals as they provide critical rehabilitation, psychology, and social work services.
Qualifications
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Helps social workers in their day to day tasks
- Discuss benefits like LTC, meals, STS, housing to determine need versus wants.
- Review and submit reports regarding any problems with a patient to supervisor.
- Comply with the patient rights and Leon Medical Centers Policies and Procedures.
- Good assessment, listening, and counseling skills.
- Perform Essential Duties as outlined above.
- Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the patients.
- Involve the patient/family in planning social service programs when possible.
- Good communication skills with excellent self-discipline and patience.
- Participate in the community planning related to the interest of the facility and the services and needs of the patient and family.
- Maintains confidentiality of all proprietary and/or confidential information.
WORK EXPERIENCE
Experience in some type of customer service role
EDUCATION
High school diploma or GED
Associate's degree in a related field preferred.
LANGUAGE SKILLS
Fluent in English and Spanish.
