This is a meaningful opportunity to make a difference by supporting patients and families during some of life’s most important moments.
Service area: King County
The Social Worker will provide a range of essential services to support the emotional, psychological, and social needs of patients and families receiving hospice and palliative care.
Conduct comprehensive psychosocial assessments
Develop and implement plans of care in collaboration with the interdisciplinary team
Provide crisis intervention and therapeutic support
Coordinate resource referrals and community services
Support patients and families navigating end-of-life care
Provide care across multiple settings, including
Private residences
Adult Family Homes
Independent & Assisted Living communities
Skilled Nursing Facilities
Qualifications
Master’s Degree MSW from an accredited program
Minimum 2 years of social work experience
1 year in Hospice and/or Home Health preferred
Strong verbal and written communication skills
Proficiency in computer charting and documentation systems
Valid driver’s license, auto insurance, and reliable transportation
