Schedule (Full-Time, Exempt)
- Saturday 7: 00 AM- 3:00 PM or 12:00 PM- 8:00 PM
- Sunday: 7:00 AM – 3:00 PM or 12:00 PM- 8:00 PM
- Monday: 4 hours for follow-ups and documentation
Key Responsibilities
- Perform patient assessments and develop individualized care plans
- Deliver skilled nursing care, including medication and treatment management
- Monitor patient progress and adjust care as needed
- Educate patients and caregivers on health management and self-care
- Coordinate with physicians and interdisciplinary teams
- Supervise Home Health Aides and LPNs
- Complete timely and accurate clinical documentation (including OASIS)
Qualifications
- Active RN license
- BSN + 1 year experience OR ASN/Diploma + 2 years experience
- Current CPR certification
- Valid driver’s license and reliable transportation
Preferred
- Home health experience
- Behavioral or mental health experience
Physical Demands & Work Environment
- Frequent standing, walking, sitting, and use of hands
- Regular bending, kneeling, and reaching
- Occasional lifting and repositioning of patients
- Ability to lift/push up to and over 25 lbs as needed
- Exposure to clinical environments and infectious materials
