Sharon Care Center is seeking a motivated and collaborative Director of Rehabilitation to lead our experienced, in-house therapy team in Centralia, Washington.
Sharon Care Center is a family-owned and operated senior care community offering Skilled Nursing, Assisted Living, and Memory Care all on one campus. This creates a unique opportunity for our rehabilitation team to support residents across multiple levels of care—from short-term skilled rehabilitation and long-term care to ongoing Medicare Part B therapy services within our Assisted Living and Memory Care communities.

What You'll Do
As Director of Rehabilitation, you will provide clinical and operational leadership for our rehabilitation program, including:
Lead and support our team of Physical, Occupational, and Speech Therapists and therapy assistants.
Oversee rehabilitation services for our Skilled Nursing Facility, including short-term rehabilitation and long-term care residents.
Manage and continue to develop Medicare Part B therapy services for residents in our on-site Assisted Living and Memory Care communities.
Manage and continue to build on our Outpatient Therapy services.
Work closely with nursing, administration, providers, and other members of the interdisciplinary team.
Support strong clinical outcomes, appropriate utilization, documentation, and regulatory compliance.
Manage staffing, scheduling, productivity, and department operations.
Mentor and develop therapists while maintaining a positive, collaborative team culture.
Identify opportunities to expand rehabilitation services and better meet the needs of residents throughout the Sharon Care Center campus.
Maintain an active clinical presence and remain connected to both residents and the therapy team.
Who We're Looking For
Candidates must be a licensed in
Physical Therapist
Occupational Therapist
Speech-Language Pathologist
A Master's degree or higher in Physical Therapy, Occupational Therapy, Speech-Language Pathology, or the applicable professional discipline is required.
Previous rehabilitation management experience in a skilled nursing or post-acute setting is preferred—but we are willing to train the right person.
