Health Link Home Health and Hospice is a leading provider of comprehensive home health services across the San Francisco Bay Area and East Bay for over 20 years. We are seeking a compassionate, organized, and team-oriented Full-Time Medical Social Worker (MSW) to serve patients in Walnut Creek and surrounding communities.

Responsibilities
- Conduct psychosocial assessments during patient admission
- Evaluate social, emotional, and environmental factors impacting care
- Collaborate on interdisciplinary care plans
- Provide counseling, resource linkage, and discharge planning support
- Consult with physicians and care teams regarding patient needs and care adjustments
- Maintain timely and accurate clinical documentation
- Supervise Social Work Assistants as needed
Requirements
- Current California Licensed Clinical Social Worker (LCSW) license
- Valid California Driver’s License
- Reliable transportation for home visits
- CPR and First Aid certification
- Ability to travel throughout Walnut Creek and surrounding East Bay areas
