Hacienda Healthcare is seeking compassionate, dependable, and dedicated individuals to join our team as Certified Nursing Assistants (CNAs) and Caregivers/Client Care Advocates (CCAs) in our Intermediate Care Facility (ICF) located in Phoenix, Arizona.

In this rewarding role, you will provide hands-on support and care to individuals with intellectual and developmental disabilities, helping them achieve their highest level of independence, health, and quality of life. Day Shift (6:00 AM - 6:30 PM)

Full-Time, 36 Hours per Week

Night Shift (6:00 PM - 6:30 AM)

What You'll Do

Direct Client Care

Assist residents with activities of daily living (ADLs), including:

Bathing
Grooming
Dressing
Feeding
Toileting
Repositioning and mobility assistance

Promote resident dignity, choice, and independence.
Support residents in social, recreational, and community activities.
Escort and transport residents to appointments and activities as assigned.
Participate in on-campus and off-campus resident activities.
Ensure residents' personal belongings and equipment are maintained and ready for daily use.

Clinical & Support Responsibilities

Measure and record vital signs (CNA responsibilities).
Observe and report changes in resident condition to nursing staff.
Assist with individualized care plans and active treatment programs.
Follow all infection control and safety procedures, including proper use of PPE.
Maintain a clean, safe, and sanitary environment.

Documentation & Compliance

Complete required documentation accurately and timely.
Follow Individual Support Plans (ISPs) and client-specific programs.
Maintain compliance with required certifications and training.
Attend mandatory staff meetings and educational sessions.
Adhere to all Hacienda Healthcare policies and procedures.

QualificationsFor CNA Applicants

Current Arizona Certified Nursing Assistant (CNA) certification in good standing.
High school diploma, GED, or equivalent experience preferred.
Previous experience in a healthcare, long-term care, rehabilitation, or developmental disability setting is a plus.

For Caregiver / Client Care Advocate Applicants

High school diploma or GED preferred.
Previous caregiving, direct care, healthcare, behavioral health, or customer service experience preferred but not required.
Compassionate attitude and desire to support individuals with developmental and intellectual disabilities.

Additional Requirements

Ability to successfully complete required background screening.
Fingerprint Clearance Card (or ability to obtain).
Current CPR/First Aid certification or ability to obtain during onboarding.
Ability to meet the physical demands of providing direct care.
Strong communication and teamwork skills.
Ability to work flexible schedules, including weekends and holidays as needed.
