About the Role
Guardian Angel Home Care is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our growing home health team in the Grand Rapids area. In this role, you will provide high-quality, patient-centered care in the comfort of patients' homes while working alongside a supportive interdisciplinary team.

Responsibilities
- Provide skilled nursing care in patients' homes under the direction of the Registered Nurse and physician
- Monitor patient condition, vital signs, and overall health status
- Administer medications and treatments as ordered and within the LPN scope of practice
- Accurately document all visits and care provided in Homecare Homebase (HCHB)
- Communicate changes in patient condition to the supervising RN and physician
- Educate patients and families on medications, disease management, and the plan of care
- Ensure compliance with all federal, state, and agency regulations
- Collaborate with the interdisciplinary team to achieve optimal patient outcomes
Requirements
- Active Michigan Licensed Practical Nurse (LPN) license in good standing
- Graduate of an accredited practical nursing program
- Current BLS/CPR Certification
- Valid driver's license, reliable transportation, and current auto insurance
- Strong clinical, communication, and documentation skills
- Ability to work independently in a home health setting
Preferred Qualifications
- Home health or hospice experience
- Experience with Homecare Homebase (HCHB) or other electronic medical record systems
- Wound care experience
- Bilingual candidates are encouraged to apply
