Licensed Practical Nurse (LPN) – Home Health

Guardian Angel Home Care is looking for a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing Home Health team in the Akron area.
As an LPN, you'll provide one-on-one skilled nursing care in patients' homes, helping them recover and maintain their independence while enjoying the flexibility and autonomy that home health offers.

Responsibilities
- Provide skilled nursing care in patients' homes
- Assess and monitor patient conditions
- Administer medications and treatments as ordered
- Perform wound care and other skilled nursing procedures
- Educate patients and caregivers on medications, disease processes, and care plans
- Document all patient visits accurately and timely using HCHB
- Communicate changes in patient condition to the RN Case Manager and physician
- Collaborate with the interdisciplinary care team to ensure excellent patient outcomes
- Maintain compliance with agency, state, and federal regulations
Qualifications
- Active Ohio Licensed Practical Nurse (LPN) license in good standing
- Graduate of an accredited practical nursing program
- Current BLS/CPR certification
- Valid driver's license, reliable transportation, and current auto insurance
- Strong communication, organizational, and documentation skills
- Ability to work independently in a home health environment
Preferred Qualifications
- Previous home health or hospice experience
- Experience with Homecare Homebase (HCHB) or another EMR
- Wound care or IV therapy experience
- Bilingual candidates are encouraged to apply
