Wound Care Registered Nurse (RN)
Position Summary
CORALVITA Healthcare is seeking an experienced and compassionate Wound Care Registered Nurse (RN) to join our clinical team.
The Wound Care RN provides specialized nursing care for patients with complex wound, skin integrity, and ostomy needs in a post-acute/LTAC setting. This role assesses patients, develops and implements individualized treatment plans, provides direct wound and ostomy care, and collaborates with physicians, nursing staff, and the interdisciplinary team to promote healing and improve patient outcomes.
The Wound Care RN also serves as a clinical resource, providing education to patients, families, and nursing staff on wound prevention, treatment, skin integrity, and ostomy management.

Key Responsibilities
- Assess and manage patients with wounds, pressure injuries, ostomies, and other skin integrity concerns.
- Develop, implement, evaluate, and document individualized wound care plans and treatment interventions.
- Perform wound assessments, measurements, photography, dressing changes, and specialized wound care procedures.
- Monitor healing and changes in patient condition and collaborate with physicians and the interdisciplinary care team regarding treatment recommendations.
- Provide wound, ostomy, skin integrity, and prevention education to patients, families, and clinical staff.
- Coordinate wound care treatments, physician orders, supplies, equipment, and related documentation.
- Support infection prevention, patient safety, regulatory compliance, and quality improvement related to wound care.
Minimum Qualifications
Licensure & Certifications
- Current, valid, and active Registered Nurse (RN) license in the state of employment.
- Current BLS certification from a CORALVITA-approved provider.
- Minimum one (1) year of wound care experience required.
- Wound Care Certification preferred within six (6) months of hire.
Experience & Skills
- Experience assessing and treating complex wounds, pressure injuries, and skin integrity concerns.
- Experience with wound measurement, staging, documentation, photography, and treatment planning.
- Ostomy care and patient education experience preferred.
- Knowledge of wound prevention, infection prevention, and evidence-based wound care practices.
- Strong clinical assessment, critical-thinking, organization, and prioritization skills.
- Ability to work independently while collaborating effectively with physicians, nursing staff, and the interdisciplinary care team.
- Strong patient, family, and staff education skills.
- Working knowledge of electronic documentation systems and clinical software applications.
