JOB DUTIES AND RESPONSIBILITIES;

 Responsible for outreach contacts with homeless mentally ill and/or co-occurring diagnosed individuals

  1. Assists in the provision of referrals and other services to eligible clients.
  2. Provides documentation of outreach activities and other services to eligible clients.
  3. Provides advocacy for homeless mentally ill and/or co-occurring diagnosed consumers
  4. Assists in filing and maintenance of charts and additional records for the program.
  5. Assists Program Participants in keeping intake and psychiatric evaluation appointments.
  6. Other duties as assigned

 

MINIMUM REQUIREMENTS:

You must have lived experience with homelessness (which means you have experienced homelessness)
You must have lived experience with mental illness!
This position is Part-Time, Mon-Fri, 8 am -1 pm, No Weekends!
Education:  High School Diploma / GED

Experience:  Current or former consumer of mental health services, homeless services, alcohol, and drug services.  Ability to follow supervision.  

Preferred Experience:  Experience working with mentally ill and or dually diagnosed adults.  Knowledge of community resources.

Salary Range:  $10.00 to $12.00 per hour.
