Senior Community Health Partner

Job Description:

The Senior Community Health Partners provides support to the Community Health Partners as well as support integrated social care for members with complex health needs (social, behavioral and physical).

Responsibilities:

• Provides support to both our Community Health Partners (CHPs) and Care Team Lead.

• Partners with the Care Team Lead and the Pod’s clinical team to run daily huddles.

• Provides guidance, coaching, and training around engagement, care planning, and longitudinal care.

• Maintain a panel of members; acts as a point person for these members and partners with them longitudinally in attaining their goals.

Work Experience:

• 3+ years of progressive experience in care coordination, community health, health promotion, or a related field and years 6+ months experience managing/leading people

Education:

• Bachelor's degree

The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location. The expected salary range for this position is: 

$32.69 - $42.07 Hourly
