Job DetailsJob Location: Cleveland - CLEVELAND HEIGHTS, OH 44118Position Type: Full TimeSalary Range: $70,000.00 - $90,000.00 Salary/yearTravel Percentage: NoneJob Shift: DayDescription: The Outreach Registered Nurse will be primarily stationed in an all-men’s club to do STI testing, give out condoms and marketing materials for Central Outreach Wellness Center and maintain connectivity between the club and Central Outreach Wellness Center clinics.

General Duties and Responsibilities

Utilize the nursing process to assess, plan and implement, and evaluate patient care.
Collects, analyzes, and interprets information from healthcare members and documents as necessary.
Collect patient health history.
Counsel and educate patients on treatment plans pertaining to HCV, HIV, STI’s, MAT and Narcan training.
Notify patients of positive and abnormal results and coordinate treatment and care immediately.
Support clinicians with any assigned duties, i.e., prescription refills etc.
Understand and educate patients on HARM reduction techniques.
Read and interpret patient information, making treatment decisions where appropriate.
Consult with clinicians to decide on the best treatment plan for patients.
Research ways to improve healthcare processes and patient outcomes through data obtained/analytical tasks/and charting tasks related to patient information.
Communicate with patients about their status and condition through emails, phone, and face to face conversations.
Perform administrative tasks such as creating patient charts and ordering supplies.
Perform phlebotomy and label appropriately per office protocols.
Oversee routing labs/immunizations/testing.
Collect samples including urine, throat swab and rectal swab as ordered.
Report results and contacts to Outreach team supervisor.
Maintain relationship with marketing department to get supplies of condoms and marketing materials as needed.

Be able to work with a team of medical assistants, clinicians, and administrative staff.

Qualifications
Active Registered Nurse license in OH

Active CPR Certification

Emphasis on HARM reduction
Always maintain HIPAA compliance.
Make sure to understand the employee handbook and follow all rules and guidelines outlined in the handbook.

Knowledge of basic computer skills, Excel, Word and how to use Electronic Medical Records
Experience with patient centered care
Punctuality and impeccable attendance
Strong communication skills with both professionals and patients from all demographics
Non-judgmental approach to patient care
Technical skills with required office equipment
Patience and great attention to detail
Completely understand the healthcare privacy laws as outlined in HIPAA.
