Position Summary:

The Behavioral Health Clinical Case Manager provides administrative and clinical coordination of behavioral health services by conducting intake assessments for all new referrals, scheduling patients with the appropriate therapist, and assessing level of care needs. This role is critical in helping patients access timely, appropriate mental health care and in ensuring smooth coordination between the Behavioral Health team, medical providers, and external resources.


KEY FUNCTIONS AND RESPONSIBILITIES: 

  • Conducts intake screenings for all behavioral health referrals.
  • Determines acuity level and appropriateness for internal services or referral to external providers.
  • Schedules patients with the appropriate behavioral health clinician based on clinical need, urgency, and availability.
  • Provides referrals and community resources for patients when care needs exceed clinic capacity.
  • Maintains accurate and timely documentation in the Electronic Medical Record (preferably eClinicalWorks)
  • Monitors referral outcomes and follows up as needed to ensure connection to care
  • Serves as liaison between behavioral health, primary care, and other departments.
  • Educates patients and families on behavioral health services, access points, and support options.
  • Participates in regular case review and departmental meetings and contributes to care planning.
  • Other duties may be assigned.

MINIMUM QUALIFICATIONS:

  • Bachelor’s degree in social work, Psychology, or related field, or commensurate experience required. Master’s a plus.
  • Minimum of 2 years’ experience in a behavioral health or clinical case management setting.
  • FQHC experience a plus.
  • Prior experience with intake, scheduling and triage of mental health referrals.
  • Proficiency with Electronic Medical Records. Proficiency eClinicalWorks, preferred.
  • Must be attentive to detail.
  • Excellent interpersonal and writing skills.
  • Bilingual (Spanish/English) preferred.
