As the Harm Reduction Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:
- Conduct outreach activities to engage the target population
- Identify community partners to engage PWUD who are unaware of their status or untreated
- Implement harm reduction strategies aimed at helping individuals minimize their immediate health and safety risks
- Provide enrollment into the agencies Syringe Exchange Program
- Facilitate brief assessments with the target populations
- Through review of the assessment, identify community based services to provide client linkage
- Facilitate linkage to services through escort as necessary
Harm Reduction Case Manager Minimum Salary: $42,000
Harm Reduction Case Manager Maximum Salary: $42,000
Harm Reduction Case Manager Schedule: Mon – Fri 8:30am – 4:30pm
Qualifications:
- High School Diploma or GED
- 1-3 years of relevant experience, Human Services
- Must have valid Driver's License and access to reliable vehicle
