About the Nancy Van Vessem Center for Healthy Aging
The Nancy Van Vessem Center for Healthy Aging supports Capital Health Plan members age 55 and older through an outpatient primary care model focused on prevention, access, and coordinated care.
Position Summary
The Nancy Van Vessem Center for Healthy Aging is seeking a Nurse Practitioner or Physician Assistant to support a busy older adult primary care practice through acute care appointments and electronic health record inbox management. This role improves timely access for members age 55 and older while supporting safe management of chronic conditions, medication questions, test results, transitions of care, and urgent clinical concerns.
Key Responsibilities
- Evaluate and treat older adult patients during acute care, same-day, and short-notice appointments, as well as routine follow-up visits for chronic conditions.
- Document patient encounters, clinical decisions, communications, and follow-up plans accurately and timely in the electronic health record.
- Collaborate with physicians, clinical staff, reception staff, caregivers, and other care team members to support continuity of care.
- Identify patients who require physician review, escalation, emergency evaluation, specialty referral, or additional follow-up.
- Assist with management of electronic health record inbox tasks, including patient and caregiver messages, clinical questions, test results, medication refill requests, forms, and follow-up needs.
- Recognize concerns specific to older adults such as polypharmacy, falls, cognitive changes, functional decline, frailty, transitions of care, and caregiver support needs.
- Support quality improvement initiatives, access goals, patient satisfaction efforts, and care coordination priorities within the practice.
Qualifications
- Current licensure as an Advanced Practice Registered Nurse or Physician Assistant in the State of Florida, or eligibility to obtain Florida licensure.
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- National certification appropriate to training and specialty.
- Geriatrics, internal medicine, or urgent care experience preferred.
- Comfort using an electronic health record to manage patient and caregiver communications, results, medication refill workflows, documentation, and team-based care tasks.
- Strong clinical judgment, communication skills, organizational ability, and commitment to patient-centered, age-friendly care.
- Ability to work independently within scope of practice while collaborating effectively with supervising or collaborating physicians and the broader care team.
Practice Environment
This outpatient-based role is part of a respected primary care practice for adults 55 and older with caring support staff, onsite diagnostic services, laboratory access, and in-house clinical resources. The position contributes directly to patient access, efficient workflows, and coordinated care for older adults and their caregivers.
