Key Responsibilities

Patient Care & Rehabilitation: Provide prescribed occupational therapy services and skilled procedures to minimize physical disabilities and help patients reach their optimum level of independence.

Assessment & Care Planning: Conduct initial assessments, develop and modify maintenance plans, perform 30-day reassessments, and collaborate with multidisciplinary home care personnel on care planning.

Physician Collaboration: Consult directly with physicians regarding changes in patient condition or treatment plans.

Qualifications
-
Passion: A genuine drive to serve others and help them thrive.

Education: Graduate of an accredited Physical Therapist Assistant (PTA) program (CAPTE, APTA, CAHEA, or AMA approved).

Licensure: Current Physical Therapist Assistant license in the state of practice (Required).

Certifications: CPR Certification (Required).
-
Experience: 1 year of home health experience preferred.

Pay: $38.00 – $45.00 per hour

Schedule: Day shift, Monday to Friday (8:00 AM – 5:00 PM); weekends as needed
