Key Responsibilities

Patient Care & Rehabilitation: Provide prescribed occupational therapy services and skilled procedures to minimize physical disabilities and help patients reach their optimum level of independence.

Assessment & Care Planning: Conduct initial assessments, develop and modify maintenance plans, perform 30-day reassessments, and collaborate with multidisciplinary home care personnel on care planning.

Physician Collaboration: Consult directly with physicians regarding changes in patient condition or treatment plans.

Competitive Pay: $30.00 – $42.00 / hour
- Schedule: Monday – Friday (Weekends as needed

Requirements & Qualifications

Education & Certification:
- Graduate of an accredited Occupational Therapy program (CAHEA/AMA/AOTA).
- Passed or eligible for the AOTA National Certification Exam.
- Graduate of an accredited Occupational Therapy program (CAHEA/AMA/AOTA).

Licensure: Current Occupational Therapist license in the state of practice.

Experience:
- Minimum of 1 year of experience in an acute care setting required.
- 1 year of Home Health experience strongly preferred.
- Minimum of 1 year of experience in an acute care setting required.

Other: Access to reliable transportation.
