πΏ What You’ll Do
As an RN Case Manager, you’ll coordinate and oversee the interdisciplinary plan of care for an assigned caseload of hospice patients, ensuring they receive compassionate, high-quality care wherever they call home.
You’ll serve as a key clinical partner for patients, families, physicians, and the interdisciplinary hospice team throughout the patient’s hospice journey.
π©Ί Key Responsibilities
π Manage an assigned caseload of hospice patients and families.
π©Ί Complete comprehensive patient assessments and ongoing evaluations.
π Develop, implement, and update individualized plans of care.
π Support effective pain and symptom management.
π€ Coordinate care with physicians and members of the interdisciplinary team.
ποΈ Provide compassionate education, guidance, and emotional support to patients and families.
π©βοΈ Supervise Hospice Aides and Licensed Practical Nurses as appropriate.
π» Maintain accurate, complete, and timely electronic medical record documentation.
π Communicate changes in patient condition and care needs to the appropriate clinical team members.
π± Participate in interdisciplinary meetings and quality improvement initiatives.
π Provide patient-centered hospice care wherever patients call home.
β Deliver exceptional end-of-life care while upholding Big Bend Hospice’s mission and values.

Qualificationsπ Qualifications
We’re Looking for Someone Who Has
π©Ί A current, valid Florida Registered Nurse (RN) license.
π₯ A minimum of one year of inpatient nursing experience OR previous hospice, home health, or long-term care experience.
π¬ Excellent communication and relationship-building skills.
π§ Strong clinical assessment, critical-thinking, and problem-solving abilities.
π€ The ability to collaborate effectively with physicians, patients, families, caregivers, and members of the interdisciplinary team.
π Strong organization and time-management skills with the ability to manage an assigned patient caseload.
π A valid Florida driver’s license, reliable transportation, and current automobile insurance.
π A passion for serving patients and families with compassion, dignity, and professionalism.
