๐Ÿ’™ Job Summary

As a Clinical Hospital Liaison, you’ll have the opportunity to make a meaningful difference for patients and families during some of life’s most important and vulnerable moments. Working directly within the hospital setting, you’ll serve as a trusted resource for patients, families, case managers, healthcare providers, and hospital staff as they explore and transition to hospice care.

In this highly collaborative role, you’ll help identify patients who may benefit from hospice services, provide education and guidance, coordinate referrals and admissions, and help ensure patients and families experience a smooth and compassionate transition of care. You’ll work closely with hospital teams and our interdisciplinary hospice professionals to coordinate services, address barriers, and connect patients with the care and resources they need.

๐Ÿ“‹ Duties & Responsibilities

Respond to direct requests for assistance, referrals, and education within assigned hospitals.*

Utilize informal and impromptu opportunities to educate, support, and develop solution-driven resolutions that strengthen positive partnerships with hospital resources and contacts.*

Provide support and direction to facility staff in delivering nursing services to patients and families who may benefit from hospice services.*

Coordinate case management of patient services and medical care for individuals who may qualify for hospice services.*

Work collaboratively within an interdisciplinary care setting to teach, assist, and support patients and families throughout the dying process.*

Participate with and provide feedback to the Team Manager and other department personnel in the development and implementation of ongoing hospice-focused education and outreach initiatives.*

Work closely with BBH staff and healthcare providers to coordinate the admission of General Inpatient (GIP) patients to our inpatient units.

Partner with hospital staff throughout the discharge-planning process, including coordination of transportation, medications, durable medical equipment (DME), and other patient needs.*

Conduct daily visits with case managers within assigned hospitals to inquire about potential referrals and identify opportunities to connect eligible patients and families with hospice services.*

Participate in cross-training with Professional Relations Liaisons in the field.*

Host quarterly Lunch & Learn educational sessions with case managers at assigned hospitals.*

Utilize the Customer Relationship Management (CRM) tool to accurately document outreach efforts, contacts, activities, and outcomes.*

Perform other duties as assigned.

Bachelor’s degree in healthcare or a related field required.

Current State of Florida or eligible Compact RN, LPN, MSW, or COTA license required.

Previous experience in hospice or palliative care preferred.

Experience working collaboratively with interdisciplinary healthcare teams preferred.

Demonstrated ability to communicate effectively and build strong professional relationships within a healthcare environment.

This position requires regular movement throughout the hospital and patient-care environment. The employee must be able to safely perform the following physical activities, with assistance when indicated:

Standing: 66–100% of the shift.

Walking: 66–100% of the shift.

Sitting: 0–33% of the shift.

Lifting: Up to 50 lbs. as needed.

Carrying: Up to 25 lbs. as needed.

Pushing/Pulling: Up to 250 lbs. on wheels, with assistance as needed.

Climbing: 1–5 flights of stairs as necessary.

Bending, squatting, kneeling, and reaching: As necessary while performing job responsibilities or providing patient care.

Rotating shoulders: As necessary to perform job responsibilities.

Crawling: If necessary to safely perform job responsibilities.
